纳尼？寨卡病毒感染会导致GBS！

 

6家医院共有68例格林-巴利综合征患者，42例患完成了血清学研究，使用RT-PCR检测血液、脑脊液、尿液中ZIKV，还进行了抗登革热病毒抗体检测。

 

共有66例患者（97%）发生格林-巴利综合征之前有ZIKV感染症状，ZIKV感染症状到格林-巴利综合征症状的时间间隔中位数为7天（四分位距为3到10）。

 

68例格林-巴利综合征患者中，50%的患者存在双侧面瘫。46例患者行神经传导及肌电图检查，36例（78%）患者与急性炎性脱髓鞘性多发性神经病表现一致。

 

42例通过RT-PCR检测ZIKV的患者中，17例（40%）结果阳性。绝大多数RT-PCR结果阳性的标本是尿标本（16/17），3份脑脊液标本结果阳性。

 

42例格林-巴利综合征患者中的18例(43%)，其ZIKV感染的存在临床及免疫学证据。42例患者中的20例(48%)有类感染发作。

 

所有患者的登革热病毒感染测试阴性。哥伦比亚ZIKV感染暴发期间，ZIKV感染与格林-巴利综合征之间的联系得到了证据支持。

Abstract:

 

BackgroundZikavirus(ZIKV)infectionhasbeenlinkedtotheGuillain-Barrésyndrome.FromNovember2015throughMarch2016,clustersofcasesoftheGuillain-BarrésyndromewereobservedduringtheoutbreakofZIKVinfectioninColombia.WecharacterizedtheclinicalfeaturesofcasesofGuillain-BarrésyndromeinthecontextofthisZIKVinfectionoutbreakandinvestigatedtheirrelationshipwithZIKVinfection.MethodsAtotalof68patientswiththeGuillain-BarrésyndromeatsixColombianhospitalswereevaluatedclinically,andvirologicstudieswerecompletedfor42ofthepatients.Weperformedreverse-transcriptase-polymerase-chain-reaction(RT-PCR)assaysforZIKVinblood,cerebrospinalfluid,andurine,aswellasantiflavivirusantibodyassays.ResultsAtotalof66patients(97%)hadsymptomscompatiblewithZIKVinfectionbeforetheonsetoftheGuillain-Barrésyndrome.ThemedianperiodbetweentheonsetofsymptomsofZIKVinfectionandsymptomsoftheGuillain-Barrésyndromewas7days(interquartilerange,3to10).Amongthe68patientswiththeGuillain-Barrésyndrome,50%werefoundtohavebilateralfacialparalysisonexamination.Among46patientsinwhomnerve-conductionstudiesandelectromyographywereperformed,theresultsin36patients(78%)wereconsistentwiththeacuteinflammatorydemyelinatingpolyneuropathysubtypeoftheGuillain-Barrésyndrome.Amongthe42patientswhohadsamplestestedforZIKVbyRT-PCR,theresultswerepositivein17patients(40%).MostofthepositiveRT-PCRresultswereinurinesamples(in16ofthe17patientswithpositiveRT-PCRresults),although3samplesofcerebrospinalfluidwerealsopositive.In18of42patients(43%)withtheGuillain-Barrésyndromewhounderwentlaboratorytesting,thepresenceofZIKVinfectionwassupportedbyclinicalandimmunologicfindings.In20ofthese42patients(48%),theGuillain-Barrésyndromehadaparainfectiousonset.AllpatientstestedwerenegativefordenguevirusinfectionasassessedbyRT-PCR.ConclusionsTheevidenceofZIKVinfectiondocumentedbyRT-PCRamongpatientswiththeGuillain-BarrésyndromeduringtheoutbreakofZIKVinfectioninColombialendssupporttotheroleoftheinfectioninthedevelopmentoftheGuillain-Barrésyndrome.(FundedbytheBartMcLeanFundforNeuroimmunologyResearchandothers.)
